Patient Forms
Upon entering our office our receptionist will greet you and welcome you as a member of our family. For your convenience, you may download your new patient paperwork to your home computer and complete them before your appointment. You may also wait until you come in for your appointment to get the paperwork and fill them out at that time. The paperwork provides us with general information about yourself and your condition. If you have health insurance, please bring your insurance card and we will then verify your benefits and eligibility.

Consultation
Next you will have a consultation with the doctor to discuss your health-related problems, concerns, and potential treatment options. This is where you tell us your story, what happened, where it hurts, etc. Your first visit is designed for the doctor to learn more about you, your condition and your expectations to determine how chiropractic care will help you meet your goals.
Examination
After your consultation, the doctor will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurological, orthopedic, postural, and physical examinations. The doctor will also preform a health history , This examination with help us to objectively evaluate all the involved tissues (bones, muscles, joints and neurology etc.) in order to determine the most accurate diagnosis possible.
X-Ray Studies
Your specific condition may require us to order x-rays or other imaging studies to get a full evaluation of a patient. X-rays help us rule out more serious conditions or assist us in developing the most effective treatment plan for you. Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.
